Summer Shore

Registration

To finish your Summer Shore registration, please download and complete the forms below. One completed set is required for each child you register for Summer Shore camps.

In order for your registration to be complete, you must submit all required camper forms PLUS payment by check to Shore Country Day School. All checks should be made out to Shore Country Day School. You may e-mail the completed forms to jnorris@shoreschool.org.

Summer Camps: Required Forms

  • Certificate of Immunizations OR documentation of exemption
  • Camper's current medical history which lists allergies, required medications, and any health conditions or impairments which may affect the camper's activities while attending camp. The medical history must be signed by a parent or guardian, or by a licensed health care provider. 
  • A record of a physical examination within the past 18 months (required only for sports camps)
